Ljubljana, 12 November 2024 – The Ministry of Cohesion and Regional Development has approved EU funding for the project Integrated care for people with chronic kidney disease (CKD) for improving health literacy and reducing health inequalities – Let’s prevent CKD.

As part of this EU-financed project, the National Institute of Public Health of the Republic of Slovenia and the University Medical Centre Ljubljana will work together to launch a prevention programme for people diagnosed with CKD at primary level and design integrated clinical pathways for managing persons with CKD. Thus, all structures and health services at various levels of the health care system and beyond will be integrated. This will pave the way towards better care for persons with CKD and their self-care. Moreover, the project will help improve accessibility to care and equal treatment of persons with CKD in Slovenia.
